Community Integration

At Passionate Hands Home Health Agency, we believe that everyone deserves to be an active, valued member of their community. Our Community Integration services are designed to support individuals with intellectual and developmental disabilities in building meaningful connections, developing life skills, and enjoying a vibrant, fulfilling lifestyle beyond the home.

Our team works one-on-one with each individual to explore social, recreational, educational, and volunteer opportunities that align with their interests and goals. Whether it’s attending community events, joining a local club, visiting the library, or volunteering at a neighborhood organization, we help individuals engage in activities that foster independence, confidence, and a sense of belonging.

We are a certified provider of the Community Integration and Habilitation (CIH) Waiver, Family Supports Waiver (FSW) and Money Follows the Person – CIH Transfer. These waivers allow us to deliver personalized support that encourages community involvement and personal growth.
